Step Two:	Choose the consultation service which is right for you. For all services, you will complete a comprehensive Health History, Lifestyle, and Diet Questionnaire. From this information I will create your personal pregnancy website, including My Profile, with all your unique facts and figures, My Pluses, a summary of factors having a positive influence on your pregnancy, My Diet Plans,with my suggestions and recommendations. The Complete Dietary Assessment includes all of the assessments described above, plus My Weight Gain, which outlines the optimal pattern of weight gain based on your height, pre-pregnancy weight, and number of babies you are carrying (singleton, twin, triplet, or quads). This advice is based on the successful program described in When You're Expecting Twins, Triplets, or Quads and the recommendations in Program Your Baby's Health: The Pregnancy Diet for Your Child's Lifelong Well-being (see Book Shelf for more info). The cost for this one-time service is $50. For individuals seeking advice regarding Fetal Reduction, this special service is available to help you weigh the risks of carrying triplets or higher-order multiples (quads or quintuplets) based on your health, medical and obstetrical history, diet, and lifestyle. In addition to the assessments described above, I will outline the risks for prematurity and other complications, with and without reduction, in your specific case. The cost for this one-time service is $50.
Step Three:	Pay for your consultation services online using secure credit card processing. Once you have chosen your consultation service, indicate how you will pay for the service, and fill in the credit card information necessary to process the purchase. Check with your insurance company, as medical nutrition therapy services may be covered. The results of each of the assessments can also be sent to your obstetrician. If you would like your physician or midwife to have this information, simply check the appropriate box and tell me his or her name, address, telephone number, and email address (if available).
